Ground rod spacing bit me after 14 years on the job

Installed a new service panel last month near Madison and skipped checking the old ground rod depth, just assumed it was fine since it was in place. Later measured it and the rod was only buried 6 feet instead of the code 8, so I had to dig it up and drive in a second one. Anybody else ever trust an existing ground rod without testing it and then get flagged on inspection?
