Why does nobody talk about root flare and how many trees are planted too deep?

I was out on my route last Tuesday delivering mail and a homeowner was out digging up a dead maple they planted three years ago. He showed me the root ball and there was a solid six inches of soil piled up on top of the main roots. I told him that's probably why it died. Then I went home and checked my own oak tree I planted five years back and sure enough I had buried the root flare too. I always thought you just dig a hole and plop the tree in and that's it. Now I see half the trees in my neighborhood have that telltale straight trunk going into the ground like a telephone pole. Who else has had to dig up and fix a tree they planted wrong?

Why not just dig a wider hole too instead of a deep one?

Disagree. Wide holes don't fix a tree planted too deep.
