My rear derailleur adjustment went totally sideways last night

I was trying to dial in the indexing on my 10-speed after a cable swap, following the Park Tool video exactly. After three turns of the barrel adjuster, the chain jumped off the cassette and got jammed between the spokes. It took me an hour to untangle everything and I had to straighten a bent derailleur hanger. I thought small tweaks were safe, but I guess not. Has anyone else had a simple adjustment cause a major failure like this?

It's wild how a tiny turn can lead to a huge mess. I see that same pattern with small software updates breaking my whole workflow.
